You have received a conditional offer of employment with TEAM Private Family Services ("TEAM"). With this, we would like to formally introduce ourselves and invite you to provide us with the necessary information to begin the hiring process.

Who is TEAM?

While TEAM will become your official Employer of Record, the duties of your job and the services you provide will not change. TEAM offers the following to all employees:

• Timely and accurate payroll processing;
• Proper withholdings and contributions towards government benefits (e.g. Social Security, Medicare);
• Access to health benefits and 401(k) retirement savings (dependent upon eligibility);
• Access to an Employee Portal for easy, secure access to electronic pay stubs and W-2s; and
• A dedicated service team to assist with any payroll or other requests.

Why am I required to complete this Enrollment Form?

TEAM must gather several forms and employee information to serve as your Employer of Record, accurately process your payroll, and abide by legal requirements. To ensure a safe and comfortable working environment for everyone and that TEAM is following consistent hiring practices, background checks are required for all employees, including employees already providing services and those related to the recipient of services.
